root-cause-analyze

root-cause-analyze is a skill for Claude Code, Codex from microsoft/cat-agent-skills. It costs 82 tokens per session (432 once invoked), scanned A, original, MIT.

A structured root-cause analysis for a quality problem. It ranks recurring cause categories using a Pareto analysis and builds an evidence-based 5-Why chain, where each why must be supported by records.

In plain words
What is it for?
It helps investigate nonconformances, identify the main recurring causes, document a 5-Why chain, and flag systemic problems.
Why use it?
It replaces unsupported guesses with a repeatable review of complaints and cause evidence. It also tests whether the problem is systemic and rejects operator error as the sole explanation for recurring defects.

About the project

microsoft/cat-agent-skills is a static website that catalogs reusable instruction sets and related packages for AI agents. People use it to search, filter, rate, and download skills for Cowork, Copilot Studio, and Scout, along with Copilot plugins and Scout automations. The catalogue entries are the skills, instructions, plugins, and settings displayed by the site.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00082 $0.00432
Opus 5.5 $0.00033 $0.00173
Sonnet 5.5 $0.00016 $0.00086
Haiku 4.5 $0.00008 $0.00043

Security

Grade A, and why

root-cause-analyze scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured today.

The scan reads SKILL.md. This mod also ships 2 executable files (scripts/pareto.py, scripts/rca_tree.py), listed below but not scanned — reading those needs a real analyzer, not pattern matching.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

What it actually says

Root Cause Analyze

Purpose

Deterministic root cause: Pareto ranks cause categories; rca_tree selects the evidenced why-chain, applies the systemic test and the operator-error rejection.

When to use

After ncr-intake in every CAPA run.

Inputs

capa-intake.json + complaints.csv + references cause-links library.

Steps

  1. Validate against the contract.
  2. Run scripts/pareto.py --capa capa-intake.json --complaints complaints.csv --out pareto.json
  3. Run python3 scripts/rca_tree.py --pareto pareto.json --links references/cause-links.json --out rca.json from this skill's folder, using workspace paths for the Pareto input and generated output.
  4. Quote the chain verbatim. If the engine rejected 'operator error' (capa-rules.md #3.4), say so explicitly - a recurring defect is never closed on retraining alone.

Output

rca.json - the {root_cause} hop with pareto[], why_chain, systemic flag.

Grounding requirements

Every why must be evidenced in the record set (rca-methods.md); the chain cites the cause-link library and complaint window.

Constraints

  • Method is engine-only; the model never invents or reorders a why-chain, and never accepts the incident narrative's cause label over the Pareto.

Escalation / uncertainty

Engine escalations (#3.4 rejection, #2.2 depth, #7 confidence) pass through verbatim.

Files

What ships with it

8 files beside SKILL.md in the same directory: the scripts, references and assets a skill reads on demand. Not counted in the per-session cost; read them before you install if any of them is executable.

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

root-cause-analyze is a skill published in the GitHub repository microsoft/cat-agent-skills (93 stars, last pushed today), licensed MIT. It adds 82 tokens to every session and 432 once invoked, about $0.0003 per session on Opus 5.5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-10-28.
