agent-skills-form

agent-skills-form is a skill for Claude Code from MohammadShehadeh/agent-skills. It costs 56 tokens per session (1,100 once invoked), scanned A, original, MIT.

A recipe for building application forms with Zod validation, React Hook Form, shadcn UI fields, and result-based submit actions. It covers field validation, submit errors, and choosing suitable controls.

In plain words
What is it for?
Use it when creating or extending forms, validation, or submit flows, especially forms with multiple fields or validation rules.
Why use it?
It reduces inconsistent form implementations by defining validation and error handling before the user interface. It also encourages reuse of existing patterns in the codebase.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00056 $0.01100
Opus 5 $0.00028 $0.00550
Sonnet 5 $0.00011 $0.00220
Haiku 4.5 $0.00006 $0.00110

Security

Grade A, and why

agent-skills-form scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 12d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

Form

End-to-end form recipe. Markup and control rules in references/forms.md. Load agent-skills-conventions for errors, components, and hooks-state when wiring submit.

1. Decide — form library or not

Situation Approach
2+ fields, validation rules, or submit state RHF + zod — follow this skill
Single uncontrolled input (search, inline rename) Plain state or a form action — no form library

Inspect the repo: reuse an existing form pattern, schema location, and submit hook before inventing a parallel one.

2. Schema first — before markup

  • Export a zod schema colocated with the feature (lib/ or next to the form component).
  • Derive the form type: type CheckoutFormValues = z.infer<typeof checkoutSchema>.
  • List validation failures (field-level, from zod) and submit failures (server/network) separately.
  • Submit failures use SCREAMING_SNAKE ErrorKey literals — add them to the feature's constants/error-keys.ts before wiring submit (agent-skills-conventions: errors rule).

Present the schema, field list, and error keys — confirm before building UI.

3. Choose controls

Pick from the control chooser in references/forms.md. Reuse shadcn/ui primitives from the project's UI layer — never raw div + space-y-* form markup.

Need Control
Simple text Input
Dropdown Select
Searchable dropdown Combobox
Boolean in settings Switch
Boolean in a form Checkbox
Single choice, few options RadioGroup
Toggle 2–5 options ToggleGroup
Multi-line Textarea
OTP InputOTP

Buttons inside inputs: InputGroup + InputGroupAddon. Related checkboxes/radios: FieldSet + FieldLegend.

4. Markup — FieldGroup / Field

  • 'use client' on the form component leaf only.
  • Wrap fields in FieldGroupFieldFieldLabel + control + optional FieldDescription.
  • Invalid/disabled: data-invalid / data-disabled on Field and aria-invalid / disabled on the control.
  • Connect RHF: register or Controller/FormField — match what the repo already uses.
  • Submit errors: render from errorKey at display time — never hardcoded strings.

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.
