Nexus: Skill for Claude Code

.claude/skills/devflow/agents/triage/SKILL.md

defect-aggregation is a skill for Claude Code from MT-gar/Nexus. It costs 52 tokens per session (960 once invoked), scanned A, original, MIT.

A defect intake and triage workflow that collects bugs from issue reports, logs, user feedback, and monitoring alerts. It merges similar reports and assigns severity levels.

In plain words
What is it for?
It helps ingest bug reports, find and merge likely duplicates, filter the defect list, and prioritize work for root-cause analysis.
Why use it?
It prevents duplicate bug reports from cluttering the work queue and gives the team a cleaner list of problems to investigate.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00052 $0.00960
Opus 5.5 $0.00021 $0.00384
Sonnet 5.5 $0.00010 $0.00192
Haiku 4.5 $0.00005 $0.00096

Measured 1mo ago against content hash fbcabb058a9b, method: parsed. Prices are Anthropic first-party input rates as of 2026-10-07, from the pricing page.

Security

Grade A, and why

defect-aggregation scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 1mo ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

What it actually says

Skill:defect-aggregation(缺陷聚合)

Schema

输入:

{ title: string, description?: string, source: "issue"|"log"|"user_feedback"|"alert"|"manual",
  severity?: "critical"|"high"|"medium"|"low", dedup_threshold?: number (0-1,默认 0.6) }

输出(ingest_defect):

{ defect_id: string, merged: boolean,
  merged_into?: string,   // merged=true 时:被合并到的规范缺陷 ID

Array<{ defect_id, title, severity, merged }>,
  建议: { 待处理缺陷数, 优先级排序 } }

list_defects 缺陷看板查询(含按 status/severity/source 过滤)
update_defect 定级分诊(status=triaged)、后续环节回写(rca_task_id/fix_task_id)
claim_task / update_task / wait_for_work 任务生命周期(基础设施)

  • 去重误判(merged 但实为不同缺陷)→ 用 ingest_defect(dedup_threshold=0) 强制新建,并报告 manager

  • ✅ 允许:ingest_defect / list_defects / update_defect(仅限定级与关联回写)

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

defect-aggregation is a skill published in the GitHub repository MT-gar/Nexus (2 stars, last pushed 1mo ago), licensed MIT. It adds 52 tokens to every session and 960 once invoked, about $0.0002 per session on Opus 5.5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-09-20.
