Getting it into your agent
One page per mod, every tool's command on it. A separate URL per tool would split the same page into five that compete with each other.

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.
| Model | Per session | Once invoked |
|---|---|---|
| Fable 5.1 | $0.00064 | $0.00642 |
| Opus 5 | $0.00032 | $0.00321 |
| Sonnet 5 | $0.00013 | $0.00128 |
| Haiku 4.5 | $0.00006 | $0.00064 |
Grade A, and why
handoff-prompt scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 10d ago.
A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.
Nothing flagged
None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.
How it starts
The opening of the file, as written. The whole thing — 61 lines — stays where its author put it; the contents beside it link to each section on GitHub.
Handoff Prompt
Generate a self-contained prompt for a new Claude Code session to execute work from the current conversation.
When to Use
- User says "give me a prompt for a new session"
- User says "handoff prompt" or "clean context prompt"
- Planning/brainstorming is done, user wants fresh context for implementation
- User wants to continue work after
/clear
Process
-
Gather from current conversation:
- Plan file path and design spec path (if any)
- 2-3 sentence summary of what to implement
- Starting point (which task, what to do first)
- Key decisions and caveats the new session must know
- Build/test/lint commands
-
Write a prompt with these sections:
- What to do (1-2 sentences)
- Files to read first (plan, spec — with full paths)
- Summary (what the plan does, in brief)
- Start by (first concrete action)
- Key context (bullet list of decisions, caveats, gotchas that aren't in the plan files)
-
Output the prompt in a fenced block so the user can copy-paste it.
Template
Implement [feature/change] for [project context].
- **Design spec:** `[path]`
- **Implementation plan:** `[path]`
**Summary:** [2-3 sentences of what the plan does]
**Start by:** [First concrete action — e.g., "Read both files, then execute Task 1 (revert commit X). Create a feature branch first."]
**Key context:**
- [Decision or caveat not obvious from the plan]
- [Another one]
- [Build/test commands if non-standard]
Rules
- The prompt must be self-contained — the new session has zero context from this conversation
- Reference file paths, not conversation content — the new session can read files but can't see our chat
- Include only context that ISN'T already in the plan/spec files — don't duplicate
- Do NOT restate rules already in global/project CLAUDE.md, AGENTS.md, or GEMINI.md. The new session loads these automatically. Examples to omit: branch naming conventions, commit message rules (no Co-Authored-By, no Claude footer), worktree locations, plan file locations, "never commit to main", subagent rules. If you're tempted to write a "Conventions" or "Ground rules" section that just echoes global preferences, delete it.
- Keep it under 300 words — long prompts waste the new session's context
- Don't include implementation details — that's what the plan file is for
What this file has done since we first saw it
Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

handoff-prompt is a skill published in the GitHub repository muhimu/claude-plan-lifecycle (2 stars, last pushed 9d ago), licensed MIT. It adds 64 tokens to every session and 642 once invoked, about $0.0003 per session on Opus 5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-08-31.
