Escrever no canvas do Nib
O diagram.json é o canvas do usuário — trabalho real, centenas de nós. Ele é last-write-wins: sem lock, sem merge, sem compare-and-swap embutido. Quem grava por último ganha, e o app sobrescreve o documento inteiro a cada clique do humano.
Isso não é teoria. Nesta base, um agente escreveu no arquivo enquanto o humano gerava um diagrama novo; as duas escritas carimbaram a mesma version 51 e o trabalho do agente evaporou. As regras abaixo existem por causa disso.

1. Nunca escreva no arquivo direto. Nada de Write, Edit, python, POST /api/diagram. Um hook (PreToolUse) barra isso, mas a regra vale mesmo se o hook falhar. Use as ferramentas:

2. Sempre expectedVersion. Leia com get_diagram, use a version que veio. Se o humano mexeu no meio, você recebe CONFLITO — releia e refaça sobre o estado novo, em vez de atropelar. Sem isso, sua escrita ou some, ou apaga a dele.
3. batch_apply, não N chamadas. Cada escrita é uma janela em que o humano pode te sobrescrever. Dez add_node = dez janelas; um batch_apply = uma, atômica (tudo ou nada). Dentro do lote, dê um ref a cada nó novo e use esse ref como source/target das arestas do mesmo lote — não precisa saber o id final.
