neura-action-card

neura-action-card is a skill for Claude Code, Codex from neurarelay/relay-action-card. It costs 58 tokens per session (299 once invoked), scanned A, original, MIT.

A template and review procedure for preparing consequential agent actions as structured Action Cards before they run.

In plain words
What is it for?
Use it before API writes, customer messages, refunds, payments, workflow changes, data exports, permission changes, and deployments.
Why use it?
It makes the proposed action, target, evidence, authority, and risks explicit so a Decision Receipt can be reviewed first.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00058 $0.00299
Opus 5 $0.00029 $0.00150
Sonnet 5 $0.00012 $0.00060
Haiku 4.5 $0.00006 $0.00030

Measured 12d ago against content hash 2bc45b35fc8e, method: parsed. Prices are Anthropic first-party input rates as of 2026-09-12, from the pricing page.

Security

Grade A, and why

neura-action-card scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 12d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

What it actually says

Neura Action Card

Use this skill before consequential agent actions. Create an Action Card v0.1 draft that can be sent to Neura Relay for a Decision Receipt before the developer-owned system executes anything.

Workflow

  1. Identify the proposed action, acting agent, target resource, affected object, evidence refs, rule refs, and risk category.
  2. Draft an Action Card using templates/action-card.v0.1.json.
  3. Keep all payloads refs-only. Do not include private customer content, secrets, credentials, token values, raw policy text, or proprietary payloads.
  4. Tell the developer that Relay returns the Decision Receipt before execution and that the developer-owned system keeps downstream execution.
  5. If authority is delegated, include only refs and posture in context.authorityContext.

Output

Return:

  • concise action summary
  • Action Card v0.1 JSON
  • missing refs or authority context
  • recommended Relay command or API path
  • reminder that no downstream execution should happen until the Decision Receipt is reviewed

For examples, read examples/support-reply.md or examples/crm-update.md only when needed.

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.
