docs-review

docs-review is a skill for Claude Code from nexus-substrate/nexus-agents. It costs 127 tokens per session (1,925 once invoked), scanned A, original, MIT.

Uma revisão baseada numa grelha de 100 pontos para avaliar documentos técnicos, como RFCs, ADRs, READMEs e guias de arquitetura. É uma auditoria de leitura que produz problemas prioritários, não uma reescrita.

In plain words
What is it for?
Analisar um ficheiro, URL ou diretório de documentos; extrair a estrutura, contar palavras e parágrafos, verificar citações e referências, e ordenar os problemas por gravidade.
Why use it?
Torna mais consistente a avaliação da qualidade de documentação e revela falhas de estrutura, clareza, evidência e referências. Evita reescrever um documento antes de saber o que precisa de melhorar.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00127 $0.01925
Opus 5 $0.00063 $0.00962
Sonnet 5 $0.00025 $0.00385
Haiku 4.5 $0.00013 $0.00193

Security

Grade A, and why

docs-review scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 7d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

Docs Review Skill

When to apply

Use when scoring an existing technical doc — RFC, ADR, README, CLAUDE.md, architecture doc, blog-style technical post. Read-only; produces a punch list, not a rewrite. The active counterpart is docs-rewrite which runs review → research → rewrite → validate.

Trigger keywords: review docs · audit docs · score this doc · doc quality · docs review · doc audit.

Inputs

  • A single file path, OR
  • A URL the operator wants reviewed (fetched + reviewed in-place), OR
  • A directory (batch mode — score every doc, sort by score)

Workflow

Step 1 — Read the doc + extract structural facts

Mechanical extraction (no judgment yet):

  • Heading structure (H1 / H2 / H3 with hierarchy)
  • Paragraph count + word counts per paragraph
  • Sentence-length distribution (for burstiness analysis below)
  • Citations present (any inline (source, year) form)
  • Info-gain markers ([ORIGINAL DATA] / [PERSONAL EXPERIENCE] / [UNIQUE INSIGHT])
  • Code blocks with language tag
  • Internal ./other-doc.md and #anchor references
  • Frontmatter (title / description / tier / keywords)

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

It adds 127 tokens to every session and 1,925 once invoked, about $0.0006 per session on Opus 5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-08-30.
