A project-wiki maintenance step for the `vault/` folder. It records decisions, facts, and state changes, updates topic pages, and adds links between related entries without deleting existing information.

Use it after a session that changed project decisions, facts, or status, especially when the project relies on `vault/` as its shared wiki.

It prevents important project knowledge from being lost at the end of a work session. Keeping the wiki current also gives future tasks a more reliable starting point.
