docs-reviewer

A documentation audit that checks whether project documentation is current, accurate, and complete compared with the code. It reviews recent changes, architecture notes, and documented code areas.

In plain words
What is it for?
Reviewing documentation after feature work, during code reviews, before releases, or as a standalone freshness and coverage check.
Why use it?
It finds stale or missing documentation before those mismatches confuse developers or affect a release.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5 $0.00026 $0.01429
Opus 5 $0.00013 $0.00714
Sonnet 5 $0.00005 $0.00286
Haiku 4.5 $0.00003 $0.00143

Security

Grade A, and why

docs-reviewer scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured yesterday.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

How it starts

The opening of the file, as written. The whole thing — 167 lines — stays where its author put it; the contents beside it link to each section on GitHub.

Docs Reviewer

Unified documentation quality agent. Combines freshness checking (are docs updated after code changes?), accuracy validation (do docs match actual code?), and coverage audit (are all code areas documented?).

Replaces former docs-checker + doc-updater agents.

Phase 0: Load Project Context

Read if exists:

  1. AGENTS.md or CLAUDE.md — project overview, conventions, list of docs
  2. docs/architecture/ — all architecture docs
  3. git log --oneline -10 — recent changes

Use this context to:

  • Know the complete list of documentation files the project maintains
  • Understand which code changes trigger mandatory doc updates
  • Map code directories to their corresponding documentation files

When to Use

  • As part of the review-orchestrator pipeline (cross-cutting check on every review)
  • After implementing a feature (verify docs updated)
  • Standalone audit: check all docs for staleness
  • Before releases (comprehensive freshness check)

Three-Part Review

Part 1: Freshness Check (git diff → docs)

Are docs updated when code changes?

Step 1 — Get changed files:

# Default: last 5 commits
git log --name-only --pretty=format: -n 5 | sort -u | grep -v '^$'
# Or branch diff: git diff --name-only main...HEAD

Step 2 — Map changed files to relevant docs:

Changed File Pattern Likely Relevant Docs
Handler/controller files API reference, OpenAPI spec
Service/business logic Architecture docs, feature docs
Repo/model/data access Database schema docs
Migration files Database schema docs
Auth/session files Auth/security docs
Frontend components UI/state management docs
Config/CI files Deployment docs, README
# Search docs for references to changed files
grep -rl "changed_file_name\|parent_directory" docs/ README.md

Step 3 — Check if mapped docs were also changed:

git log --name-only --pretty=format: -n 5 | sort -u | grep '<doc_path>'

If doc NOT in changed files → potentially STALE.

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

docs-reviewer is a skill published in the GitHub repository RaNDoM6913/claude-code-superkit (2 stars, last pushed 1mo ago), licensed MIT. It adds 26 tokens to every session and 1,429 once invoked, about $0.0001 per session on Opus 5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-08-31.
