freshness-triage

freshness-triage is a skill for Claude Code, Codex from reem-sab/tech-writer-prompts. It costs 100 tokens per session (942 once invoked), scanned A, original, MIT.

A documentation review process that compares a documentation page with its Git history and the current source files it describes. Git history is the record of changes made to files over time.

In plain words
What is it for?
Use it to investigate potentially outdated documentation and define a concrete check for each suspicious claim.
Why use it?
It highlights claims that may no longer match the code, while ranking which ones deserve attention first.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00100 $0.00942
Opus 5 $0.00050 $0.00471
Sonnet 5 $0.00020 $0.00188
Haiku 4.5 $0.00010 $0.00094

Security

Grade A, and why

freshness-triage scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 12d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

Freshness Triage

Cross-references a documentation page against its git log and the current source it documents to rank which specific claims are most likely stale. Confirmed contradictions come first, then claims whose underlying source changed after the page was last touched, each with a concrete check to confirm it.

Inputs

  • {{PAGE}}: The documentation page to check for staleness.
  • {{GIT_LOG}}: Git log entries (commit date, message, changed files) for the page and/or the source it documents.
  • {{SOURCE}}: The current contents of the source file(s) the page documents (config, constants, code).

How to apply

Substitute each {{PLACEHOLDER}} above with the user's actual content, then follow the prompt below exactly — including its output format and its stated limits.

You are triaging PAGE for staleness by cross-referencing its claims against GIT_LOG (the commit history for the page and/or its source) and SOURCE (the current state of what it documents). You are not confirming anything is actually wrong — you're ranking which specific claims deserve a human's attention first, because the evidence suggests they might have drifted from reality.

Method

  1. Extract the concrete, checkable claims from PAGE — specific numbers, named behaviors, field names, defaults, limits — not general prose.
  2. For each claim, check whether SOURCE currently supports it. A claim that directly contradicts SOURCE is the highest-risk finding there is.
  3. For claims SOURCE doesn't directly settle, use GIT_LOG: if the relevant file was changed by a commit dated after the page's last edit (or after whichever commit most plausibly wrote that claim, if that's inferable from GIT_LOG), treat the claim as at elevated risk even without direct contradiction — something nearby changed after the docs were last touched.
  4. Claims neither contradicted by SOURCE nor touched by a later commit in GIT_LOG are lower priority — the evidence doesn't suggest they moved.

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.
