ascended

ascended is a skill for Claude Code, Codex from Septemc/MistakebookSkill. It costs 37 tokens per session (334 once invoked), scanned A, original, MIT.

A shortcut that starts Mistakebook Ascended Mode, a workflow for reviewing past corrections and stored project knowledge before answering. It is designed to help the agent learn from repeated mistakes.

In plain words
What is it for?
Starting the Ascended Mode review process when a user requests it or when the same answer has been rejected repeatedly.
Why use it?
It gathers relevant previous mistakes, notes, and memory so the same issue is less likely to be handled incorrectly again.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00037 $0.00334
Opus 5 $0.00018 $0.00167
Sonnet 5 $0.00007 $0.00067
Haiku 4.5 $0.00004 $0.00033

Measured 7d ago against content hash c5676806474c, method: parsed. Prices are Anthropic first-party input rates as of 2026-09-06, from the pricing page.

Security

Grade A, and why

ascended scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 7d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

What it actually says

Ascended Entry

This is the Codex skill-chip wrapper for Mistakebook Ascended Mode.

Before acting, read these source files so this wrapper keeps the same behavior as the core Mistakebook protocol:

  • ../../skills/mistakebook/SKILL.md
  • ../../skills/mistakebook/references/activation-patterns.md
  • ../../skills/mistakebook/references/storage-and-scope.md
  • ../../skills/mistakebook/references/archive-schema.md
  • ../../skills/mistakebook/references/ascended-mode.md

When this skill is invoked:

  1. Treat the current task as a Mistakebook Ascended Mode case
  2. First output exactly: 我现在会根据我见过最有效的方法来处理这个问题,我将检索我的所有知识库,我现在什么都不缺了!
  3. Then follow the existing Mistakebook Ascended Mode workflow
  4. If scripts/mistakebook_cli.py exists, prefer: python scripts/mistakebook_cli.py context --host codex --project-root . --scope both --mark-retrieval
  5. Before giving the new answer, explain why previous handling failed and which knowledge sources you reviewed

This wrapper exists for Codex UX only. It should behave like the existing Ascended Mode protocol, but enter through a skill chip instead of a prompt body expansion.

Files

What ships with it

1 file beside SKILL.md in the same directory: the scripts, references and assets a skill reads on demand. Not counted in the per-session cost; read them before you install if any of them is executable.

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

ascended is a skill published in the GitHub repository Septemc/MistakebookSkill (23 stars, last pushed 4mo ago), licensed MIT. It adds 37 tokens to every session and 334 once invoked, about $0.0002 per session on Opus 5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-08-30.
