Getting it into your agent

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.
| Model | Per session | Once invoked |
|---|---|---|
| Fable 5.1 | $0.00075 | $0.07347 |
| Opus 5.5 | $0.00030 | $0.02939 |
| Sonnet 5.5 | $0.00015 | $0.01469 |

Grade A, and why
grounding scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 1mo ago.
A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.
Nothing flagged
None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.
How it starts
The opening of the file, as written. The whole thing — 404 lines — stays where its author put it; the contents beside it link to each section on GitHub.
Grounding Skill
Default engine: lexical mode
The grounder runs a frozen-weight logistic (LexicalVerdict) over 13-18 signals selected by the effort tier. High is the default tier (18 features, includes MT cross-lingual via argos CTranslate2 int8). Per-layer thresholds (--threshold, --bm25-threshold) affect the cascade fallback path only; in lexical mode the verdict comes from the logistic. Semantic (--semantic) and NLI are opt-in extras, unchanged.
Effort tiers (CPU-only, no extra install required)
low- 13 features; fastest; no language detection; English-onlymedium- 16 features; adds lingua language detection + WordNet antonym contradictionhigh- 18 features; adds argos MT for cross-lingual claims (translates non-English claim to EN then recall-scores); default
Set via lexical_effort in config_document_processing.yaml or --effort CLI overlay.
Three-step grounding chain
Grounding a document = three-step chain, not just step 2:
extract-claims— enumerate load-bearing claims from document intoclaims.json(heuristic, lossy — review before grounding).ground— score every claim against source(s) with lexical-mode CLI (frozen-weight logistic over 13-18 signals; + optional--semantic); read verdicts, apply verdict rules; writegrounding-report.md.check-consistency— check document against itself for divergences grounding structurally can't see (42 usershere vs50 usersthere;dev/test/stagingvsdev/staging/prod); writeconsistency-report.md.
Document-grounding run produces both grounding-report.md and consistency-report.md. Single claim (no document) → run only step 2's single-claim form (document-processing ground). Many documents → validate runs whole chain per client. Always run the CLI - canonical operational grounder; generative interpretation only an on-top layer for semantic claims after CLI ran.
validate skill wraps this whole chain, adds tone/style/length/format compliance layer on top; process skill invokes this skill from its Verify & Ground phase; update skill calls it as its mandatory closing step.
What this file has done since we first saw it
Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

grounding is a skill published in the GitHub repository stellarshenson/claude-code-plugins (4 stars, last pushed today), licensed MIT. It adds 75 tokens to every session and 7,347 once invoked, about $0.0003 per session on Opus 5.5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-09-05.
