neurologist

neurologist is a skill for Claude Code from supportersimulator/3-surgeons. It costs 18 tokens per session (434 once invoked), scanned A, original, MIT.

A local-model health and review assistant that checks system components, questions proposed changes, and examines its own capabilities. A local model runs on your own machine or infrastructure.

In plain words
What is it for?
Use it for health checks, major architecture or security decisions, and situations where you want assumptions or shortcuts challenged.
Why use it?
It can expose unavailable services, broken state or evidence storage, stale GPU locks, and weaknesses in a proposed approach before work continues.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00018 $0.00434
Opus 5 $0.00009 $0.00217
Sonnet 5 $0.00004 $0.00087
Haiku 4.5 $0.00002 $0.00043

Security

Grade A, and why

neurologist scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 10d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

What it actually says

Neurologist

The Neurologist (Qwen3-4B local) provides three capabilities: health monitoring, corrigibility skepticism, and self-introspection.

Pulse (Health Check)

Run before critical operations to verify the entire system is healthy.

When to pulse:

  • Session start (part of probe)
  • Before operations that depend on all surgeons being available
  • After infrastructure changes (model swaps, endpoint changes)
  • When something feels off (slow responses, unexpected results)

What it checks:

  • llm_health — can the neurologist respond?
  • state_backend — is the state store operational?
  • evidence_store — is the evidence database accessible?
  • gpu_lock — is the GPU lock stale (dead PID holding it)?

MCP tool: neurologist_pulse_tool CLI: 3s neurologist-pulse

Challenge (Corrigibility Skeptic)

The neurologist challenges assumptions about proposed changes. Use this before committing to an approach.

When to challenge:

  • Major architectural decisions
  • Changes that affect safety or security
  • When you're confident but haven't tested the counter-position
  • Before implementing an approach that bypasses existing patterns

What you get back:

  • claim — what was assumed to be true
  • challenge — the counter-argument
  • severity — critical, worth_testing, or informational
  • suggested_test — how to verify the counter-position

MCP tool: neurologist_challenge_tool CLI: 3s neurologist-challenge "proposed change"

Introspect (Capability Self-Report)

Ask each surgeon to honestly report what they can and cannot do.

When to introspect:

  • After changing models or endpoints
  • When a surgeon gives unexpected results
  • To calibrate confidence in surgeon outputs

MCP tool: introspect_tool CLI: 3s introspect

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

neurologist is a skill published in the GitHub repository supportersimulator/3-surgeons (2 stars, last pushed 3mo ago), licensed MIT. It adds 18 tokens to every session and 434 once invoked, about $0.0001 per session on Opus 5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-08-31.
