review-loop

review-loop is a skill for Claude Code from supportersimulator/3-surgeons. It costs 20 tokens per session (549 once invoked), scanned A, original, MIT.

A rule set that chooses how many review rounds a three-reviewer examination should receive based on the task’s risk.

In plain words
What is it for?
Use it before cross-examination of configuration changes, features, refactors, security work, schema changes, APIs, or architecture.
Why use it?
It helps match review effort to the number of files, reversibility, security exposure, data impact, and external connections involved.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00020 $0.00549
Opus 5 $0.00010 $0.00275
Sonnet 5 $0.00004 $0.00110
Haiku 4.5 $0.00002 $0.00055

Security

Grade A, and why

review-loop scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 5d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

Review Loop Mode Selector

Purpose

Before any 3-surgeon cross-exam, determine the appropriate review depth mode based on task risk signals.

Modes

Mode Iterations When
single 1 Config changes, docs, small fixes, <=2 files
iterative up to 3 Multi-file changes, new features, refactors, 3-10 files
continuous up to 5 Security, schema, API, architecture, >10 files

Risk Assessment

Evaluate these dimensions. Highest-risk dimension wins:

Dimension single iterative continuous
Files changed 1-2 3-10 >10
Reversibility Fully Partially Hard to reverse
Security None Internal External/auth
Data impact Read-only Schema-preserving Schema-changing
External coupling None Internal APIs Public APIs

Project-Level Awareness

Some projects warrant deeper review by default. Check:

  • .3surgeons/config.yaml in project root for review.depth
  • Accumulated review outcome weights via 3s review-weights show
  • Project complexity signals: cross-file deps, security code, submodules

For sustained development on complex projects (e.g., ContextDNA), continuous mode is the natural default.

Intent Mapping (Conversational)

User says Maps to
"quick review", "just a glance" single
"thorough review", "check carefully" iterative
"loop until satisfied", "full depth", "keep going" continuous
"3-surgeons full" continuous

Auto-Depth Behavior

Based on auto_review_depth config:

  • off: Always use config default or CLI flag
  • suggest: Recommend mode with reasoning, wait for confirmation
  • auto: Apply learned weights, user can override anytime

Exit Conditions

  • Consensus >= 0.7 on "all issues addressed" → exit loop
  • Max iterations reached without consensus → escalate to human with unresolved summary
  • User override at any point → respect immediately

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

review-loop is a skill published in the GitHub repository supportersimulator/3-surgeons (2 stars, last pushed 3mo ago), licensed MIT. It adds 20 tokens to every session and 549 once invoked, about $0.0001 per session on Opus 5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-08-31.
