Getting it into your agent
One page per mod, every tool's command on it. A separate URL per tool would split the same page into five that compete with each other.

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.
| Model | Per session | Once invoked |
|---|---|---|
| Fable 5.1 | $0.00041 | $0.01015 |
| Opus 5.5 | $0.00016 | $0.00406 |

Grade A, and why
knowledge-review scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 23d ago.
A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.
Nothing flagged
None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.
How it starts
The opening of the file, as written. The whole thing — 48 lines — stays where its author put it; the contents beside it link to each section on GitHub.
Knowledge Review
You are the reviewer for one knowledge base pass. Code enforces every hard rule; you supply judgement as typed decisions with a confidence. Works from any agent: it is plain CLI plus JSON files.
Steps
-
Extract. Follow the
knowledge-extractskill for changed sources (squadrant knowledge ingest <kb> --json, thenapply). -
Get the packet.
squadrant knowledge review <kb> --jsonlistsproposals(each with the live rule it replaces and any rules it conflicts with, plus priorities),stalerules,decidedcode-vs-doc conflicts, and open escalations. -
Decide each item. Write
decisions.json:{ "decisions": [ { "item": "biz.rounding.vnd-half-up@1a2b3c4d5e6f", "decision": "approve", "confidence": 0.92, "justification": "New revision states the same obligation with a tighter scope." }, { "item": "stale:sec.logging.no-passwords", "decision": "approve", "confidence": 0.9, "justification": "Source section was removed." }, { "item": "stale:biz.x", "decision": "reject", "confidence": 0.9, "justification": "Still stated in section 4.", "loc": "p.9 §4" } ], "pairs": [ { "a": "x", "b": "y", "relation": "duplicate", "keep": "x", "confidence": 0.9, "justification": "..." } ] }decision:approve|reject|merge-into:<ruleId>|escalate. Forstale:items, approveretires, rejectkeeps the rule active (optionally updateloc).confidenceis 0..1. At or aboveautoApproveConfidence(packet field, default 0.8) code applies it; below, it is escalated. Be honest: a low number is useful.- Set
"flag": truewhen you want a human regardless. pairs(full pass only): classify the candidate pairs asduplicate | refines | conflicts | unrelated.
-
Code vs doc. Where a rule is contradicted by the project's code, add
"codeConflict": {"project": "...", "files": ["src/x.ts"], "summary": "..."}to that rule's decision (use the live rule id asitemif there is no proposal). Do not decide who is right. Code dates the conflict against the doc by git; if the code is newer it always goes to the operator, who is not asked again unless the doc or the anchored code changes. If the doc is newer it is recorded as a code violation. -
Apply.
squadrant knowledge review <kb> --apply decisions.json. -
Close.
squadrant knowledge reconcile <kb> [--full] --finishwrites REPORT.md, advances the schedule and sends the captain a one-line count.
What this file has done since we first saw it
Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

knowledge-review is a skill published in the GitHub repository tu11aa/squadrant (2 stars, last pushed yesterday), licensed MIT. It adds 41 tokens to every session and 1,015 once invoked, about $0.0002 per session on Opus 5.5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-10-11.
