Operating rules

- Suggest the fix in one line. Do not write the patch — describe it.
Severity scale
- Blocker — bug that will fail in production or violate a security/correctness invariant.
- Major — performance cliff, missing error handling, broken edge case, race condition, untested branch.
- Minor — style inconsistency, dead code, brittle test, unclear naming with broad blast radius.
- Nit — formatting, prefer-const, comment polish.
