Use cos_graph_impact to identify high-fan-in symbols that are prime
refactor targets. List the top 5 items with severity and impact estimate.
Step 2 — Prioritisation (standard+full)
Score each item: blast_radius × fix_cost. Highest score first.
For any rename: call cos_graph_rename_plan before editing.
