cochrane

cochrane is a skill for Claude Code from aliildan/cochrane-mcp. It costs 46 tokens per session (351 once invoked), scanned A, original, MIT.

A research tool for the Cochrane Library, a collection of systematic reviews and clinical trial records. It finds reviews and trials and returns their details, including plain-language summaries and study questions.

In plain words
What is it for?
Use it to search clinical evidence, read systematic reviews, inspect randomised trials, and answer questions about what Cochrane has found.
Why use it?
It saves time when checking what high-quality medical research says about a treatment or health question. It also helps separate evidence reviews from individual trials.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00046 $0.00351
Opus 5 $0.00023 $0.00176
Sonnet 5 $0.00009 $0.00070
Haiku 4.5 $0.00005 $0.00035

Security

Grade A, and why

cochrane scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 9d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

What it actually says

Cochrane Library

Use the cochrane_* MCP tools for evidence from the Cochrane Library.

Workflow

  1. cochrane_suggest_terms (optional) to refine vague terms.
  2. cochrane_search — default type: "review" (CDSR systematic reviews). The response includes typeCounts for all content types (review, protocol, central, editorial, specialcollections, cca), so you can tell the user how much evidence exists per type, then re-search a different type.
  3. cochrane_get_details with a result's doi for the structured abstract, Plain Language Summary, and PICO (reviews) or the trial record (central).

Tips

  • searchField options: title-abstract-keyword (default), record-title, abstract, author, keyword, all-text, source, doi, accession-number, cochrane-group.
  • Prefer reviews for synthesised evidence; use central (trials) for primary studies (often thousands).
  • Cite the DOI and link from urls.html.

Setup caveat (Cloudflare)

The server mints a cf_clearance cookie via a real Chrome. If a tool returns a Cloudflare-challenge error, the user must ensure Chrome is reachable (COCHRANE_CDP_ENDPOINT) or solve the one-time challenge in the auto-launched browser window.

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

cochrane is a skill published in the GitHub repository aliildan/cochrane-mcp (1 stars, last pushed 2mo ago), licensed MIT. It adds 46 tokens to every session and 351 once invoked, about $0.0002 per session on Opus 5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-08-31.
