report

report is a skill for Claude Code, Codex from jamillazarev/multica-ops. It costs 32 tokens per session (348 once invoked), scanned A, original, Apache-2.0.

A reporting workflow for recording problems in a product, workspace, or skill and saving the evidence in the appropriate place.

In plain words
What is it for?
Use it to report product bugs, record repeated workspace friction, or prepare a file describing a defect in the skill or Multica.
Why use it?
It removes the need to decide immediately who caused the problem and keeps reports consistent, de-identified, and reviewable.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00032 $0.00348
Opus 5 $0.00016 $0.00174
Sonnet 5 $0.00006 $0.00070
Haiku 4.5 $0.00003 $0.00035

Security

Grade A, and why

report scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 5d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

What it actually says

Load and follow the multica-ops skill (../mops/SKILL.md), executing its /multica-ops:report flow — the procedure is in SECURITY.mdReporting a defect in this skill.

Something went wrong, or made the work harder than it needed to be. You do not have to know whose defect it is — that is the first thing this decides, and it decides it from the evidence rather than asking you to classify your own frustration.

A defect in your product is the urgent lane and belongs to /multica-ops:bug: reproduced before it is fixed, straight to Build and Review. Friction in your workspace — a flow that fought you, a step that repeats — becomes a field note in _ops/FIELD-NOTES.md, one line, swept at the checkpoints PLAYBOOKS names — session end, /multica-ops:status, before a release cut; a second occurrence earns an issue with both occasions named. Friction in this skill or in Multica itself is assembled from evidence — version, flow, symptom, the task record, the state of the files involved — de-identified, and written whole to a file outside your repository — written first, with anything missing marked unknown, never held back waiting for a field, with its path said out loud and the ways to send it named. Publishing is yours: the file is made here, you post it.

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.
