adr-writing

adr-writing is a skill for Claude Code from KhaledSaeed18/dotclaude. It costs 101 tokens per session (992 once invoked), scanned A, original, MIT.

An Architecture Decision Record (ADR) writing guide. An ADR is a short, dated record of an important engineering choice, the alternatives considered, and the consequences for future developers.

In plain words
What is it for?
It helps record choices about databases, frameworks, public APIs, data formats, error handling, module boundaries, and authentication.
Why use it?
It preserves the reasoning behind difficult, broad, surprising, or disputed decisions instead of leaving future engineers to guess why the system works that way.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00101 $0.00992
Opus 5.5 $0.00040 $0.00397
Sonnet 5.5 $0.00020 $0.00198
Haiku 4.5 $0.00010 $0.00099

Security

Grade A, and why

adr-writing scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 25d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

How it starts

The opening of the file, as written. The whole thing — 81 lines — stays where its author put it; the contents beside it link to each section on GitHub.

An ADR records a decision at the moment it was made, with the reasoning that was available then. It is not documentation of the system (that changes) and not a design document (that precedes the decision). Its reader is an engineer two years from now asking "why is it like this, and can I change it?"

When to write one

Write an ADR when a decision is:

  • hard to reverse (a database, a framework, a public API shape, a data format), or
  • cross-cutting (affects how most code is written: error handling convention, module boundaries, auth model), or
  • surprising (a reader would assume the other option), or
  • contested (people disagreed, and the losing option will come up again).

Do not write one for choices that are local, obvious, or cheaply reversible; a comment or a commit message serves those. A repository with 200 ADRs has stopped recording decisions and started recording activity.

Format

# 0007. Use PostgreSQL row-level security for tenant isolation

Date: 2026-09-21
Status: accepted
Supersedes: 0003
Deciders: (names or roles)

## Context

What situation forces a decision. The constraints (scale, team, deadline, compliance), the problem observed, and what happens if nothing is decided. Facts, with numbers where they exist. No solutions yet.

## Options considered

### A. Application-level filtering (current)
How it works in one or two sentences. Pros. Cons. What it costs to stay.

### B. Row-level security in PostgreSQL
Same.

### C. Schema per tenant
Same.

## Decision

One sentence: "We will use B: row-level security policies on every tenant-scoped table, enforced by a session variable set in the connection middleware."

Then the reasoning that tipped it, referring to the context's constraints. Two or three paragraphs at most.

## Consequences

What becomes easier. What becomes harder or is now forbidden (and what to do instead). Migration or rollout steps and who owns them. What would make us revisit this (the trigger for a superseding ADR).

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

adr-writing is a skill published in the GitHub repository KhaledSaeed18/dotclaude (5 stars, last pushed 2d ago), licensed MIT. It adds 101 tokens to every session and 992 once invoked, about $0.0004 per session on Opus 5.5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-09-22.
