sepsis-screening

sepsis-screening is a skill for Claude Code, Codex from langcare/langcare-mcp-fhir. It costs 105 tokens per session (3,776 once invoked), scanned A, original, MIT.

A sepsis-screening helper that calculates qSOFA, SOFA, and SIRS scores from a patient’s vital signs, laboratory results, diagnoses, and treatment records. Sepsis is a serious response to infection that can cause organ failure.

In plain words
What is it for?
Use it when infection and possible sepsis are suspected, to review relevant criteria, assess risk, and check elements of the early sepsis treatment bundle.
Why use it?
It collects the measurements used by several screening systems and records the resulting risk assessment in one place.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00105 $0.03776
Opus 5.5 $0.00042 $0.01510
Sonnet 5.5 $0.00021 $0.00755
Haiku 4.5 $0.00011 $0.00378

Security

Grade A, and why

sepsis-screening scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 2mo ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

Sepsis Screening

Overview

Calculate qSOFA, SOFA, and SIRS scores from FHIR Observation and Condition resources. Pull vitals (temperature, heart rate, respiratory rate, blood pressure, SpO2) and labs (WBC, lactate, creatinine, bilirubin, platelets, PaO2/FiO2) to generate a comprehensive sepsis risk assessment. Create a ClinicalImpression resource documenting the findings. Evaluate compliance with Surviving Sepsis Campaign hour-1 bundle elements.

FHIR Resources Used

Resource Purpose Key Fields
Observation Vitals and lab values code, valueQuantity, effectiveDateTime, status
Condition Active infections, comorbidities code, clinicalStatus, onsetDateTime
Patient Age, demographics birthDate, gender
MedicationRequest Vasopressor use, current antibiotics medicationCodeableConcept, status, authoredOn
Procedure Mechanical ventilation status code, status, performedPeriod
ClinicalImpression Output: sepsis risk assessment status, description, finding, summary

Instructions

Step 1: Retrieve Patient Demographics

Tool: fhir_read
resourceType: "Patient"
id: "[patient-id]"

Extract birthDate (calculate age) and gender. Age modifies baseline risk interpretation.

Step 2: Pull Vital Signs (Last 24 Hours)

Retrieve each vital sign category using LOINC codes. Use _sort=-date&_count=5 to get recent trends.

Temperature:

Tool: fhir_search
resourceType: "Observation"
queryParams: "patient=[patient-id]&code=8310-5&_sort=-date&_count=5"

LOINC 8310-5 = Body temperature. Flag if >38.3C (100.9F) or <36.0C (96.8F).

Heart Rate:

Tool: fhir_search
resourceType: "Observation"
queryParams: "patient=[patient-id]&code=8867-4&_sort=-date&_count=5"

LOINC 8867-4 = Heart rate. Flag if >90 bpm (SIRS) or >100 bpm.

Respiratory Rate:

Tool: fhir_search
resourceType: "Observation"
queryParams: "patient=[patient-id]&code=9279-1&_sort=-date&_count=5"

LOINC 9279-1 = Respiratory rate. Flag if >=22 (qSOFA) or >20 (SIRS).

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

sepsis-screening is a skill published in the GitHub repository langcare/langcare-mcp-fhir (71 stars, last pushed 6mo ago), licensed MIT. It adds 105 tokens to every session and 3,776 once invoked, about $0.0004 per session on Opus 5.5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-08-30.
