Getting it into your agent
One page per mod, every tool's command on it. A separate URL per tool would split the same page into five that compete with each other.

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.
| Model | Per session | Once invoked |
|---|---|---|
| Fable 5.1 | $0.00351 | $0.03662 |
| Opus 5 | $0.00176 | $0.01831 |
| Sonnet 5 | $0.00070 | $0.00732 |
| Haiku 4.5 | $0.00035 | $0.00366 |
Grade A, and why
fearless-organization scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 10d ago.
A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.
Nothing flagged
None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.
How it starts
The opening of the file, as written. The whole thing — 129 lines — stays where its author put it; the contents beside it link to each section on GitHub.
The Fearless Organization
Amy Edmondson's operating system for building teams and organizations where people take the interpersonal risks required to learn, innovate, and prevent avoidable failure. Distilled from The Fearless Organization (Wiley, 2018), Right Kind of Wrong: The Science of Failing Well (Atria, 2023 — winner of the FT Business Book of the Year), Teaming (Jossey-Bass, 2012), her HBR canon since the mid-2000s, the TEDxHGSE talk (2014), the Fearless Organization Scan (fearlessorganizationscan.com), and podcast appearances on HBR IdeaCast, Freakonomics, Hidden Brain, TED Radio Hour, Coaching for Leaders, Intentional Leader, Armchair Expert, and NeuroLeadership Institute conversations.
This skill helps the assistant think in Edmondson's method — not just recite the definition of psychological safety. It's opinionated because Edmondson is opinionated: psychological safety is NOT being nice, and it does NOT come without accountability. Present PS without the accountability axis and you produce the exact misread Edmondson has spent seven years pushing back against — the "safe space" caricature that lowers standards and stops shipping.
When this skill activates
Use this skill when the user is:
- Diagnosing why their team has gone quiet in meetings, retros, or 1:1s.
- Redesigning a post-mortem, incident retro, blameless review, or after-action report.
- Deciding how to respond to a specific failure — deploy gone bad, a project that missed, an experiment that didn't pan out.
- Thinking about a near-miss they just witnessed at work (an engineer who tried to raise a safety concern, a product decision made without dissent, a "green all around" status meeting that felt off).
- Running or designing an offsite on team culture, values, or ways of working.
- Rewriting incident-response processes, on-call handoffs, or engineering excellence rituals.
- Thinking about how to introduce psychological safety to a leadership team without triggering the "we already have a safe space" misread.
- Auditing a "we're a psychologically safe culture" claim — either their own or one made by a company they're joining or advising.
- Debugging what "learning organization" or "blameless culture" means in practice, when the org has adopted the language but not the behavior.
- Deciding how to sanction misconduct in a way that reinforces PS rather than undermining it.
- Facing pressure to lower standards in the name of "being kind" or "not creating anxiety."
- Trying to make sense of a Boeing- / Columbia- / Wells Fargo- / Volkswagen-style organizational failure.

What this file has done since we first saw it
Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

fearless-organization is a skill published in the GitHub repository marcos-sponton/frameworks-as-skills (1 stars, last pushed 1mo ago), licensed MIT. It adds 351 tokens to every session and 3,662 once invoked, about $0.0018 per session on Opus 5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-08-31.

planning-with-files
Use this by default for non-trivial multi-step work that needs persistent planning, progress tracking, or durable notes on disk. Trigger when a task will likely span multiple tool calls, research steps, verification loops, or enough context that the plan should not live only in transient chat memory.
obsidian-kb-artifacts
Use this skill for Obsidian-native formatting and derived artifacts such as Markdown formatting, wikilinks, registry tables, canvas files, optional Bases, CLI operations, and link repair. This skill does not decide knowledge routing.
fast-resume
Search local coding-agent session history and identify safe resume commands. Use when the user asks to find, inspect, continue, or recover previous work from Claude Code, Codex, Pi, or another agent indexed by fast-resume.

Copy text to clipboard with optional rich formatting. Triggers on "copy to clipboard", "copy that", "pbcopy", "copy formatted", "copy rich text".
