pubmed-nlm

pubmed-nlm is a skill for Claude Code from mayank-io/mstack. It costs 72 tokens per session (1,971 once invoked), scanned A, original, MIT.

A skill that retrieves biomedical article records from PubMed, the U.S. National Library of Medicine's research database. It can obtain metadata, structured abstracts, subject terms, and available full text through the database's API.

In plain words
What is it for?
Use it to fetch a biomedical paper for saving, reading, clipping, or summarising, including open-access full text when available.
Why use it?
It provides article information in structured form without using a browser or scraping web pages. It can resolve a PubMed ID, PMC ID, DOI, or article URL.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00072 $0.01971
Opus 5.5 $0.00029 $0.00788
Sonnet 5.5 $0.00014 $0.00394
Haiku 4.5 $0.00007 $0.00197

Security

Grade A, and why

pubmed-nlm scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 1mo ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

How it starts

The opening of the file, as written. The whole thing — 94 lines — stays where its author put it; the contents beside it link to each section on GitHub.

Fetch PubMed Article

Pull a PubMed record — bibliographic metadata, the structured abstract, MeSH indexing, and the PMC full text when the article is open access — into a directory. This skill knows nothing about vaults. It retrieves, and stops. notes:clip turns the result into a note.

No browser — this one is an API

Unlike every other skill in this plugin, do not open a browser for PubMed. NCBI publishes the entire corpus through E-utilities as XML over plain HTTP: no login, no cookies, no JavaScript, no rate-limit key for light use. There is no gstack section here because there is nothing for gstack to do.

Driving a browser at pubmed.ncbi.nlm.nih.gov would scrape a rendering of data the API hands over structured — losing MeSH qualifiers, grant numbers and author affiliations that never appear on the page — and would miss the full text entirely, which lives on a different host under a different id.

Arguments

  • $ARGUMENTS should contain: <url|pmid|pmcid|doi> [output_dir]

All four reference forms resolve to the same record:

Input Resolution
https://pubmed.ncbi.nlm.nih.gov/26041386/ direct
26041386 direct
PMC4986668 or a /pmc/articles/PMC… URL reverse-resolved to its PMID via the PMC record
10.1093/jamia/ocv046 or a doi.org URL resolved via esearch — efetch has no DOI lookup

Instructions

python3 ${CLAUDE_PLUGIN_ROOT}/scripts/pubmed_fetch.py $ARGUMENTS

The script handles everything:

  1. Parses the reference and resolves it to a PMID
  2. Fetches efetch.fcgi?db=pubmed&id=<pmid>&retmode=xml
  3. Parses metadata, authors and affiliations, the structured abstract, MeSH headings, keywords, grants and the reference list
  4. If the record carries a pmc id, fetches efetch.fcgi?db=pmc&id=<numeric>&retmode=xml for the full text
  5. Writes the raw XML (both documents), a <pmid>.json record, and a rendered <pmid>.md

Set NCBI_API_KEY to raise the rate limit from 3 to 10 requests/second, and NCBI_EMAIL to identify yourself to NCBI. Both are optional and neither is sent unless set — do not add the user's email without being asked.

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

pubmed-nlm is a skill published in the GitHub repository mayank-io/mstack (6 stars, last pushed 3d ago), licensed MIT. It adds 72 tokens to every session and 1,971 once invoked, about $0.0003 per session on Opus 5.5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-09-14.
