note-context

note-context is a skill for Claude Code, Codex from microsoft/cat-agent-skills. It costs 159 tokens per session (1,736 once invoked), scanned A, original, MIT.

A clinical note reader that turns a signed encounter note into structured, cited facts for later documentation checks. It stops when the note is unsigned or still a draft.

In plain words
What is it for?
It extracts documented problems, medicines, orders, results, claims, and other encounter facts, with citations back to the note.
Why use it?
It gives downstream work a traceable factual basis and prevents draft or unsigned notes from being used as if they were final records.

About the project

microsoft/cat-agent-skills is a static website that catalogs reusable instruction sets and related packages for AI agents. People use it to search, filter, rate, and download skills for Cowork, Copilot Studio, and Scout, along with Copilot plugins and Scout automations. The catalogue entries are the skills, instructions, plugins, and settings displayed by the site.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00159 $0.01736
Opus 5.5 $0.00064 $0.00694
Sonnet 5.5 $0.00032 $0.00347
Haiku 4.5 $0.00016 $0.00174

Security

Grade A, and why

note-context scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 2d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

Note Context

Purpose

Establish the documented, cited facts every downstream artefact and the quality-gap check depend on - or, for an unsigned note, stop the whole run and route it back to the clinician.

When to use

Start of every run - triggered by "pull the facts out of today's note", "structure this visit note", "what did the note actually document?", "start the documentation run". Also use it when the note turns out to be unsigned or a draft: this skill owns the halt-and-escalate for rule #5, so it must run so that the halt happens.

When NOT to Use

  • Drafting a referral, after-visit summary or care plan - use document-derive instead.
  • Explaining whether the note supports a code on the claim - use coding-rationale instead.
  • Computing quality-measure status - use quality-gap-check instead.
  • Writing a clinician handoff or sign-out note - use handoff-draft instead.
  • Summarising a document that is not a clinical encounter note - use the built-in docx skill instead.

Inputs

Encounter note text with its signature line; problem and medication lists; orders and results; the claim block (if any); config/quality-measures.json; base/contracts/hls.clinical-documentation-companion.v1.json.

Steps

  1. Confirm the note is signed: an electronic-signature line naming the signer and time, or note_signature.signed == true on a supplied contract. A "draft", "pended" or unsigned note fails this check. If it fails, stop here: write only the halt record (Output Format) and escalate - no encounter, no note_facts (the contract does not require encounter in an unsigned-note halt record).
  2. Extract encounter = {patient_id, visit_type, date} from the note header.
  3. Build the fact-key list - do not guess key names: a. required_fact_key of every measure in config/quality-measures.json; b. supported_by of every claim.elements[] entry; c. the document-derive facts: referral_indicated, active_medications, patient_instructions, follow_up_plan (one entry each; use the note's own wording in citation.text).
  4. For each key write "<fact_key>": {"value": <bool>, "citation": {"source": "<DocumentReference/...>", "locator": "<section, line N>", "text": "<verbatim sentence>"}}. For a single-finding key, quote the sentence that supports the fact, not the whole line. For the document-derive facts in 3c, also write statements[] with one {text, citation} entry for every relevant sentence the note documents - every medication, every instruction, every follow-up item, the referral and its reason - so nothing a downstream draft needs is lost; citation then repeats the first statement's citation. Never paraphrase a finding into existence.
  5. When the note is silent on a key, record value: false with locator: "full note reviewed" and text stating the absence - absence is a documented absence, not an omission.
  6. If the note's date is not the date the user called "today", say so in one line and use the note's date.
  7. Write encounter, note_signature and note_facts to the contract (carry contract_version and claim through unchanged) and hand off to document-derive, coding-rationale and quality-gap-check.

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

note-context is a skill published in the GitHub repository microsoft/cat-agent-skills (101 stars, last pushed yesterday), licensed MIT. It adds 159 tokens to every session and 1,736 once invoked, about $0.0006 per session on Opus 5.5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-11-06.
