to-issues

to-issues is a skill for Claude Code from quay/ai-helpers. It costs 50 tokens per session (756 once invoked), scanned A, a copy of to-issues, MIT.

A guide for turning a plan, specification, or PRD into separate issue files. A PRD is a document describing what a product or feature should do; each issue is a piece of work someone can take on.

In plain words
What is it for?
Use it to draft implementation tickets, decide which slices need human review, and prepare work for an issue tracker.
Why use it?
It makes large plans easier to implement by dividing them into end-to-end slices instead of isolated tasks for one technical layer.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00050 $0.00756
Opus 5 $0.00025 $0.00378
Sonnet 5 $0.00010 $0.00151
Haiku 4.5 $0.00005 $0.00076

Security

Grade A, and why

to-issues scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured yesterday.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

To Issues

Break a plan into independently-grabbable issues using vertical slices (tracer bullets).

Process

1. Gather context

Work from whatever is already in the conversation context. If the user passes an issue reference (issue number, URL, or path) as an argument, fetch it from the issue tracker and read its full body and comments.

2. Explore the codebase (optional)

If you have not already explored the codebase, do so to understand the current state of the code. Issue titles and descriptions should use the project's domain glossary vocabulary, and respect ADRs in the area you're touching.

3. Draft vertical slices

Break the plan into tracer bullet issues. Each issue is a thin vertical slice that cuts through ALL integration layers end-to-end, NOT a horizontal slice of one layer.

Slices may be 'HITL' or 'AFK'. HITL slices require human interaction, such as an architectural decision or a design review. AFK slices can be implemented and merged without human interaction. Prefer AFK over HITL unless the slice requires an irreversible architectural decision or external approval.

4. Quiz the user

Present the proposed breakdown as a numbered list. For each slice, show:

  • Title: short descriptive name
  • Type: HITL / AFK
  • Blocked by: which other slices (if any) must complete first
  • User stories covered: which user stories this addresses (if the source material has them)

Ask the user:

  • Does the granularity feel right? (too coarse / too fine)
  • Are the dependency relationships correct?
  • Should any slices be merged or split further?
  • Are the correct slices marked as HITL and AFK?

Iterate until the user approves the breakdown.

5. Write issue files

For each approved slice, write a markdown file in issues/ in the current directory using sequential numbering: issues/001-short-slug.md, issues/002-short-slug.md, etc. Create the issues/ directory if it doesn't exist.

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

to-issues is a skill published in the GitHub repository quay/ai-helpers (3 stars, last pushed 19d ago), licensed MIT. It adds 50 tokens to every session and 756 once invoked, about $0.0003 per session on Opus 5. A static security scan graded it A with 0 findings. It is 80% identical to to-issues, differing in 18 lines, and is treated as a copy.
