retrospective

retrospective is a skill for Claude Code from rjmurillo/ai-agents. It costs 87 tokens per session (2,824 once invoked), scanned A, original, MIT.

A structured review process for examining a completed session or task and recording what was learned.

In plain words
What is it for?
Use it to investigate failures, fill retrospective records, extract learnings, and create reusable project knowledge.
Why use it?
It helps teams find evidence, diagnose root causes with methods such as Five Whys, and turn lessons into specific follow-up actions. A retrospective is a review held after work is done.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00087 $0.02824
Opus 5.5 $0.00035 $0.01130
Sonnet 5.5 $0.00017 $0.00565
Haiku 4.5 $0.00009 $0.00282

Security

Grade A, and why

retrospective scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 2d ago.

The scan reads SKILL.md. This mod also ships 3 executable files (scripts/extract_evidence.py, scripts/run_retrospective.py, scripts/score_atomicity.py), listed below but not scanned — reading those needs a real analyzer, not pattern matching.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

How it starts

The opening of the file, as written. The whole thing — 227 lines — stays where its author put it; the contents beside it link to each section on GitHub.

Retrospective

Turn execution experience into institutional knowledge. This skill orchestrates a fixed Phase 0 through Phase 5 workflow that gathers evidence, generates insights, diagnoses root causes, decides actions, scores atomicity, and persists learnings. The long-form rubrics live verbatim in references/; this file is the orchestration contract.

This skill replaces the former retrospective agent (.claude/agents/retrospective.md). It is callable from Skill("retrospective") and from /retro fill <date> (Issue #2079). The Stop hook that once invoked it automatically was deleted in #3349, so a retrospective is written when someone asks for one.

Triggers

Trigger Phrase Operation
run a retrospective Full Phase 0..5 workflow over the given scope
retro fill Fill an unfilled auto-retro skeleton for a date
extract learnings from this session Phase 0..4 over the current session
diagnose this failure Phase 0..2 root-cause analysis, then stop
what did we learn Phase 4 atomicity-scored learning extraction

When to Use

Situation Use This Skill?
A reflect pass produced a HIGH-confidence finding Yes
main went red, or a merge was reverted or force-fixed Yes
The user corrected the work explicitly Yes
A session ended and nothing above fired No, a calendar is not a trigger
An unfilled auto-retro skeleton exists in .project-toolkit/retrospective/ Yes (fill it)
Diagnosing why a task failed (Five Whys, fishbone) Yes
Capturing a single in-conversation correction ("no", "wrong") No, use reflect
Saving a quick checkpoint with no analysis No, use /checkpoint

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

retrospective is a skill published in the GitHub repository rjmurillo/ai-agents (48 stars, last pushed today), licensed MIT. It adds 87 tokens to every session and 2,824 once invoked, about $0.0003 per session on Opus 5.5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-09-03.
