keystone-review

keystone-review is a skill for Claude Code, Codex from tacoda/keystone. It costs 18 tokens per session (305 once invoked), scanned A, original, MIT.

A review workflow that examines the current code changes through functional, security, risk, and deployment checks. It is a semantic review, meaning it judges whether the change makes sense beyond simply compiling or passing tests.

In plain words
What is it for?
Use it after automated checks like linting, type checking, tests, and builds pass, and before opening a pull request for an important change.
Why use it?
It provides a second pass for problems such as missing requirements, unsafe input handling, excessive impact, or deployment and rollback issues.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00018 $0.00305
Opus 5 $0.00009 $0.00152
Sonnet 5 $0.00004 $0.00061
Haiku 4.5 $0.00002 $0.00030

Security

Grade A, and why

keystone-review scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 9d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

What it actually says

keystone:review — semantic review of the current diff

Phase 5 of the six-phase lifecycle. Fires the four inferential review sensors against the in-progress diff:

  • review-functional — does the change implement the spec?
  • review-security — auth, input validation, secrets, injection.
  • review-risk — blast radius, data integrity, observability.
  • review-deployment — migrations, rollback, config, infra deps.

Each sensor returns a structured finding list. Semantic check — runs after computational sensors (lint / type / test / build) pass.

Canonical playbook: .charter/actions/review.md. Full discipline at .charter/guides/process/review.md.

Run

Open .charter/actions/review.md and execute every activity.

When to trigger

  • After /keystone:verify is clean.
  • Before opening a PR.
  • Anytime a change feels load-bearing and a second pass is cheap.
Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.
