human-review-guide

human-review-guide is a skill for Claude Code from tu11aa/squadrant. It costs 162 tokens per session (3,940 once invoked), scanned A, original, MIT.

A short guide for a person reviewing pull requests after automated checks have run. It focuses on decisions machines cannot reliably judge, such as intent, scope, public names, architecture, and whether tests cover the right behaviour.

In plain words
What is it for?
It helps prepare a human review gate for a group of pull requests, listing what was not covered, what needs human judgement, and which missing checks should become future automation.
Why use it?
It prevents human reviewers from repeating automated checks and highlights gaps where no machine check exists. This makes the human review focus on uncovered risks and follow-up improvements.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00162 $0.03940
Opus 5.5 $0.00065 $0.01576
Sonnet 5.5 $0.00032 $0.00788
Haiku 4.5 $0.00016 $0.00394

Security

Grade A, and why

human-review-guide scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 16d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

human-review-guide — a local guide for the human review gate

Core principle: the human gate covers only what the gates cannot see

Code and rules are already reviewed by agents and gated: crew self-review, the owner's agent review, CI, lint, guard tests, knowledge-base rule checks, a repo's PR review tool. The human gate exists only for what those gates cannot see or did not cover. Two kinds of thing qualify:

  1. Not covered — no gate looked at it in this diff.
  2. Only a human can judge it — intent and scope, public contract names, architecture trade-offs, whether the tests test the right thing, a live comparison with a design or prototype.

Everything else is dropped from the guide, and the human is never asked to redo it. The guide also records the gate gaps: things that should have been machine-checked but no gate covers, each with a suggested new gate or follow-up ticket, so a human catches it once and a machine catches it next time.

This skill produces the guide that makes that review fast.

Structure follows addyosmani/agent-skills code-review-and-quality (five axes, tests first, severity labels, common rationalizations).

Who writes it

The agent in charge of the PR(s) writes the guide itself, whichever session briefed, reviewed and drove them (normally the captain). It holds the context: why each choice was made, the review findings, the decisions taken since. Do not hand the guide, or its HTML, to a new crew, a side session or a fresh subagent; it lacks that context and produces a shallow guide. If you have rolled over or compacted, reload context first (your handoff, gh pr view N --comments, the commit history, the review findings) and then write it yourself.

LOCAL ONLY — never post the guide

The guide is for the operator. Never post it, or any part of it, on a PR, issue, tracker ticket, chat channel or any other shared place. Not as a comment, a description edit, a review body or a status update.

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.
