triage-review

triage-review is a skill for Claude Code from vergil-project/vergil-claude-plugin. It costs 96 tokens per session (792 once invoked), scanned A, original, MIT.

A human-reviewed process for sorting uncurated issue reports into existing epics, tasks, or maintenance work. It gathers triage-labelled issues across the organisation's managed repositories.

In plain words
What is it for?
Use it for a regular backlog-intake pass: collect triage issues, recommend whether to close or assign each one, and ask a human to confirm the decision.
Why use it?
It keeps incoming issues from piling up or being forgotten, while ensuring each one gets a clear outcome before action is taken.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00096 $0.00792
Opus 5 $0.00048 $0.00396
Sonnet 5 $0.00019 $0.00158
Haiku 4.5 $0.00010 $0.00079

Security

Grade A, and why

triage-review scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 11d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

Triage review

Overview

triage-capture fills a lossless net of triage-labelled issues. This skill empties it: a human-in-the-loop pass that routes each uncurated issue into the epic/task model, so the queue stays small and nothing rots. Run it on a cadence (weekly-ish) or whenever the queue feels heavy.

When to use

"review triage", "groom the backlog", "process the triage queue", "what's in triage", "let's do intake".

Workflow

  1. Collect the queue (cross-repo). Gather every triage issue across the org's managed repos and vergil-project/.github:

vrg-gh issue list --repo <owner>/<repo> --label triage \
      --json number,title,url,body

Run per repo (or via search) and assemble one list. Report the count.

  2. Disposition each issue, one at a time. Present the issue, recommend a disposition, and let the human confirm before acting. Pick exactly one:

    Disposition Action
Drop Duplicate / wontfix / already done → close it.
Assign to an existing epic It's a task in an initiative → native-link it under that epic.
Route to the standing epic Small ad-hoc work → native-link under the repo's Ad-hoc maintenance standing epic.
Promote to a new epic A real seed → start superpowers:brainstorming to create a finite epic; this issue becomes/links to it.

Prefer assigning to an existing epic over the standing bucket ("a forgotten to-do in that project").

  3. Apply the disposition:

    • Link (assign / route): vrg-epic-link --epic <owner>/.github#<EPIC> --task <owner>/<repo>#<N> to create the native sub-issue link.
    • Remove the triage label (it has entered the model): vrg-gh issue edit <N> --repo <owner>/<repo> --remove-label triage (add a kind label if missing).
    • Drop: closing an issue is a human action — agents are denied issue close. Tell the human to close it (or do it via ! vrg-gh issue close …).
  4. Stop when the queue is empty (or the human calls it). Report what was assigned, routed, promoted, and dropped.

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

triage-review is a skill published in the GitHub repository vergil-project/vergil-claude-plugin (2 stars, last pushed 8d ago), licensed MIT. It adds 96 tokens to every session and 792 once invoked, about $0.0005 per session on Opus 5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-08-31.
