severity-scoring

severity-scoring is a skill for Claude Code, Codex from yogsoth-ai/stress-test. It costs 31 tokens per session (278 once invoked), scanned A, original, Apache-2.0.

A scoring method that rates how serious each failure mode is on a scale from 1 to 10, based on its eventual impact.

In plain words
What is it for?
Use it to score failure modes in a risk or FMEA review, using the resulting ratings to support later prioritisation.
Why use it?
It gives different failure scenarios a consistent severity assessment. This makes it easier to compare consequences and focus attention on the most serious ones.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00031 $0.00278
Opus 5 $0.00015 $0.00139
Sonnet 5 $0.00006 $0.00056
Haiku 4.5 $0.00003 $0.00028

Security

Grade A, and why

severity-scoring scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 7d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

What it actually says

Severity Scoring

Rates each failure mode's severity on a 1-10 scale based on end-effect impact.

Execution

Subagent — spawned via subagent-spawning/spawn-agent.

Why Subagent

Severity assessment requires consistent calibration across all modes. Isolated context prevents anchoring bias from occurrence or detection considerations.

Input

  • failure_modes: Failure mode catalog
  • chains: Cause-mode-effect chains (for end-effect reference)

Output

  • scores: List of (failure_mode_id, severity_score, justification)
  • calibration_notes: Any scoring edge cases or assumptions

Available SOPs

Optional, no fixed order; the final leaf is always a sop.

SOP When to use
spawn-agent Spawn a customized CC subagent with full MCP tool access. Used by SOPs that declare execution: subagent.
Files

What ships with it

1 file beside SKILL.md in the same directory: the scripts, references and assets a skill reads on demand. Not counted in the per-session cost; read them before you install if any of them is executable.

Changes

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

It adds 31 tokens to every session and 278 once invoked, about $0.0002 per session on Opus 5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-09-03.
