Step 3 — Verify

Constraints
- Blameless: people don't cause incidents, systems do.
- Distinguish root cause (proximate) from contributing factors (conditions that made it worse).
- Do not invent impact numbers; mark unknowns.
- Trust runtime evidence over recollection.
