dismech: Skill for Claude Code

.claude/skills/evidence-claim-mismatch/SKILL.md

evidence-claim-mismatch is a skill for Claude Code from monarch-initiative/dismech. It costs 48 tokens per session (622 once invoked), scanned A, original, BSD-3-Clause.

A review workflow for checking whether evidence excerpts support claims in a disease record. It compares the claim's wording and qualifiers with the relevant research snippets and repairs mismatches when needed.

In plain words
What is it for?
Use it to audit evidence and claims in disease YAML entries, investigate evaluation findings, and search for additional literature when necessary.
Why use it?
It helps catch overstatements and evidence that supports only part of a claim, rather than treating an automated evaluation as the final answer.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00048 $0.00622
Opus 5.5 $0.00019 $0.00249
Sonnet 5.5 $0.00010 $0.00124
Haiku 4.5 $0.00005 $0.00062

Security

Grade A, and why

evidence-claim-mismatch scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 27d ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

Review evidence–claim mismatches

Find claims whose selected snippets do not support what the YAML asserts. Expand the literature search to include more papers where necessary. Evaluation results are leads for review, not instructions to agree with a model.

Read the claim in context

  • Read the current disease YAML and the issue discussion, including linked PRs. Issue examples are saved evaluation inputs and may lag behind edits. Skip resolved findings; do not recreate old problems to match a snapshot.
  • Use the linked saved assessments to go beyond the issue's few examples. Assess the disease/subtype and inherited context as well as the assertion's terms, descriptions, qualifiers, support/refutation, and directness. / means the whole claim; other paths identify individual aspects of that claim.
  • A snippet may support the term but not a bundled description or qualifier. Distinguish overclaiming from a claim that is simply less detailed than its source. A broader, correctly supported claim does not need to repeat every detail of the excerpt.
  • Nested claims with their own evidence are separate assertions. In particular, evidence on a pathophysiology node need not justify its downstream edges. Check those edges' evidence separately. Consider complementary evidence on the assertion before deciding that the KB claim itself is unsupported.
  • Treatment/medical-action claims may describe hypotheses about an action's effect on a mechanism; do not automatically read them as clinical recommendations.

Repair the evidence or claim

Read the cached reference to understand the passage, while asking whether the selected snippet conveys the needed support. The task is claim–snippet fit, not rating the source. Extend or replace a snippet to make its context clear; ... and [editorial context] are available when faithful to the source. Do not add an unsupported qualifier inside editorial brackets. Follow dismech-references for quoting, cache generation, and validation.

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

evidence-claim-mismatch is a skill published in the GitHub repository monarch-initiative/dismech (69 stars, last pushed today), licensed BSD-3-Clause. It adds 48 tokens to every session and 622 once invoked, about $0.0002 per session on Opus 5.5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-09-28.
