dismech: Skill for Claude Code

.claude/skills/mapping-analysis/SKILL.md

mapping-analysis is a skill for Claude Code from monarch-initiative/dismech. It costs 82 tokens per session (2,475 once invoked), scanned A, original, BSD-3-Clause.

A guide for investigating whether disease concepts from different medical vocabularies mean the same thing and cover the same scope. It distinguishes families, subtypes, symptoms, susceptibility concepts, and billing codes rather than treating similar names as equivalent.

In plain words
What is it for?
Use it to review MONDO and external-ontology mappings, subtype and grouping links, low-confidence or retracted mappings, coding mappings, proxy merges, and proposed corrections.
Why use it?
A mapping can look wrong when the two concepts have different levels of detail or purposes. This helps separate genuine mapping errors from valid differences and records the evidence for a correction.

Token cost

What it costs to keep this loaded

Counted locally with the o200k_base tokenizer, which is exact for GPT models; Claude uses its own tokenizer and its counts differ. Treat this as one consistent yardstick across the catalogue rather than a bill. Prices are per million input tokens.

ModelPer sessionOnce invoked
Fable 5.1 $0.00082 $0.02475
Opus 5.5 $0.00033 $0.00990
Sonnet 5.5 $0.00016 $0.00495
Haiku 4.5 $0.00008 $0.00248

Security

Grade A, and why

mapping-analysis scanned grade A with 0 findings against 26 rules in 11 categories — prompt injection, anti-refusal, data exfiltration, privilege escalation, supply chain, agent snooping, system-prompt leakage, SSRF and excessive agency — measured 1mo ago.

A static scan of the body, not an audit. Every finding is printed with the line that produced it so you can judge whether it matters here. A mod is markdown that instructs an agent; that is exactly why what it instructs is worth reading.

Nothing flagged

None of the 26 patterns this scan looks for appear in this file: no shell pipes, no recursive deletes, no credential paths, no hidden text, no instruction-override or anti-refusal phrasing, no agent-config snooping. That is not a guarantee, it is the absence of the things that are checkable.

Analyze mappings in dismech

Determine whether the mapped concepts have the same scope, explain the alternatives, and identify the evidence for a correction. Boomer checks consistency under supplied constraints and priors; it does not establish clinical equivalence or infer gene associations.

Start with the question and saved evidence

Identify the disease entry, subtype, grouping, or cross-source mapping at issue. Read its asserted mapping predicate and intended scope before treating a different target as an error. A disease family, genetic subtype, phenotype, susceptibility, and coding category may have similar names but different scopes.

Use the existing artifacts under analyses/boomer:

Artifact What to inspect
index.tsv, mendelian.tsv Cohort membership, grounding, and saved status
disorders/<slug>/kb.yaml Hard constraints, hypotheses, input priors, and labels
disorders/<slug>/solution.yaml, solution.md, solve.json Chosen assignment, probabilities, timeout status, input hash, and solver revision
disorders/<slug>/proxy-merges.json Annotation evidence and decisions, where generated
remaining-low-confidence/ Distinct alternatives, source context, and residual triage
proxy-merges/ Curated permissions, source catalog, original inputs, and migration manifests
runs/ Attempt records and comparisons, including failed or incomplete searches
groupings/, cross-source/, icd10/ Solver-free membership checks, direct mapping comparisons, and ICD coverage

An index row or an old solution.md is not enough to establish a current result. Check the input hash, solver revision, and completion status together. Separate current completed searches, timeouts, stale results, and results from older solvers when reporting counts or confidence distributions. Compute cohort sizes from the artifacts; historical README totals are not permanent defaults.

What this file has done since we first saw it

Hashed on every crawl. A supply-chain change to an agent config is a question of when, not whether, so the history is kept rather than the latest state alone.

mapping-analysis is a skill published in the GitHub repository monarch-initiative/dismech (69 stars, last pushed today), licensed BSD-3-Clause. It adds 82 tokens to every session and 2,475 once invoked, about $0.0003 per session on Opus 5.5. A static security scan graded it A with 0 findings. No closer match exists in the catalogue, so it is treated as the original; first seen 2026-09-21.
