Workflow

- Preflight:
inspect_variantとvalidate_refsで事前診断する。 - パッチ計画を準備し、
patch_applyの dry-run モードで差分を確認する。 error/criticalで停止。修正をsafe_fixまたはdecision_requiredに分類する。- confirm モードで適用(
